Job Overview

This is a challenging yet rewarding role that requires a strong commitment to delivering high-quality care to older adults with dementia. As a Community Care Worker for Older Adults, you will be responsible for providing support and guidance to care home staff, reviewing electronic records, and making entries.

You will also attend meetings face-to-face and on MS Teams, contributing to multidisciplinary team discussions and decisions. A full UK driving licence and vehicle for business use are required for this post, with reasonable adjustments made for disabled individuals.

Key Responsibilities

  • Support the implementation of patient care plans
  • Monitor and record individuals' mental health status
  • Report any significant changes
  • Assist individuals in developing and maintaining coping strategies

Salary and Benefits

The estimated salary for this role is £28,000 - £34,000 per annum, depending on experience. The successful applicant will also receive a range of benefits, including opportunities for career development and progression, a supportive working environment, and access to training and supervision.
