Llamau Ltd is seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will be responsible for managing a caseload of clients across the area, working autonomously and managing your own schedule.
Key Responsibilities:
- Develop and implement personalized support plans for clients, taking into account their unique needs and goals.
- Work collaboratively with local agencies, such as the local authority, specialist charities, education and training providers, probation, social services, and housing provisions, to ensure comprehensive support for clients.
- Provide emotional support and guidance to clients, helping them to overcome barriers and achieve positive outcomes.
- Conduct regular home visits and community outreach to ensure clients are receiving the support they need.
- Maintain accurate records and reports, ensuring compliance with Llamau Ltd's policies and procedures.
Requirements:
- Proven experience in support work or housing, with a strong understanding of housing options, housing law, and basic benefits.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and stakeholders.
- Non-judgmental mindset and understanding of the impacts of homelessness, substance misuse, family breakdown, domestic abuse, and trauma.
- Forward-thinking and resilient attitude, with the ability to overcome barriers and setbacks.
- Strong character, with the ability to remain calm and collected in challenging situations.
- Full UK driving license and access to a car.
