About the Role:We are seeking a dedicated and compassionate practice nurse to join our team in Brighton. The successful candidate will be responsible for delivering high-quality patient care and working closely with our healthcare professionals to achieve excellent outcomes.
Key Duties:- Conduct health assessments and develop personalized care plans for patients.
- Administer vaccinations, perform health screenings, and provide wound care as needed.
- Collaborate with healthcare teams to ensure seamless patient care and effective communication.
- Support health promotion initiatives and educate patients on managing chronic conditions.
- Maintain accurate patient records and adhere to clinical guidelines.
- RGN with current NMC registration.
- Proven experience in primary care or community settings.
- Excellent communication and teamwork skills.
- Ability to work independently and as part of a team.
