About the Role

St George's University Hospitals NHS Foundation Trust is seeking a highly skilled Neurological Care Coordinator to join its team in Wandsworth. The successful candidate will be responsible for coordinating care for patients with neurological conditions, working closely with multidisciplinary teams to deliver high-quality patient outcomes.

Job Responsibilities

  • To coordinate care for patients with neurological conditions, ensuring seamless transitions between healthcare settings.
  • To work with patients and their families to provide excellent clinical knowledge and support.
  • To develop and implement evidence-based clinical nursing practice in line with current research and guidelines.

Requirements

  • NMC Level 1 or Level 15 Registered Nurse
  • Degree (BSc) or equivalent in a health-related subject
  • Extensive relevant experience in neurology/neurosurgery (acute, inpatient, or community)
  • Ability to impart complex information to patients, relatives, and carers with different levels of understanding

Salary Information

The estimated salary for this position is £43,141 - £71,324 per annum, depending on experience.
