Are you passionate about delivering high-quality support to individuals in their own homes? We are seeking a dedicated and compassionate Healthcare Support Specialist to join our Discharge 2 Assess team.

About the Role:

  • The D2A service provides short-term support to individuals who require extra care in their homes.
  • You will assess customers' needs to identify areas where support is needed and provide person-centred care.
  • Motivate customers to achieve their goals and promote independent living.
  • Support customers with tasks around the home and personal care.
  • Shifts will be a mixture of early and late shifts between 7 am and 10 pm and alternate weekends.
  • A £500 welcome bonus is available.

About You:

  • Previous experience in caring for vulnerable adults is essential.
  • You must be sensitive, caring, and passionate about delivering a first-class service.
  • Proactive and self-motivated with excellent communication skills.
  • QCF Level 2 in Health and Social Care is required.
  • A full driving licence and access to your own vehicle are necessary for lone working and travelling between customers.

Job Benefits:

  • £500 welcome bonus on successful completion of your probationary period.
  • Double time on bank holidays and an extra £1 per hour on weekends.
  • Mileage allowance of 45p per mile.
