Role Details:

  • This is an exciting time to join our organisation following internal review and improvements.
  • You will be responsible for managing a portfolio of patients, including regular touchpoints and visits. Excellent communication and client-facing skills are essential.
  • You will work closely with patients and carers to ensure a stable team of carers is always rostered and informed.
  • You will conduct supervisions and annual appraisals for carers working for your patients.
  • You will be adept at problem-solving and ensuring continuity of care for patients.
  • All tasks must be delivered with a focus on delivering high-quality care and customer service excellence.
