In this exciting new role, you will be responsible for supporting patients to self-medicate and regulate their insulin following a stay in hospital or having been newly diagnosed. You will work closely with the multidisciplinary team to deliver high-quality care, treatment, and education to patients with diabetes and their families.

The successful candidate will have a passion for improving patient outcomes and satisfaction, and will be able to demonstrate expertise in diabetes care and education. They will also have excellent communication and interpersonal skills, and be able to build strong relationships with community healthcare providers.

Key Requirements

  1. Proven experience in diabetes care and education;
  2. Demonstrated ability to work collaboratively with multidisciplinary teams;
  3. Excellent communication and interpersonal skills;
  4. Ability to build strong relationships with community healthcare providers;
  5. Possession of relevant qualifications and certification.

Salary Details

Estimated salary: £38,000 - £48,000 per annum (dependent on experience) plus benefits.
