Key Responsibilities:

  • Provide rehabilitation services to patients in a 34-bed rehab facility, with a focus on enabling them to return home safely.
  • Conduct community-based assessments and interventions, working with patients to develop personalized plans to improve their functional abilities.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure optimal outcomes.
  • Stay up-to-date with best practices and industry developments to provide evidence-based care.

Requirements:

  • Minimum 2 years of experience as an Occupational Therapist in a rehabilitation or community setting.
  • Strong knowledge of rehabilitation principles and practices, including assessment, intervention, and outcome measurement.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Valid driver's license and access to a vehicle (for community-based work).
