Home Care Team Supervisor

About the Position
We are seeking an experienced and compassionate Home Care Team Supervisor to join our team at Cartrefi Cymru Co. As a key member of our leadership team, you will be responsible for managing a team of professionals who provide high-quality support services to adults with learning disabilities in Conwy.
Job Summary
The successful candidate will have a strong background in social care, preferably with experience supporting adults with learning disabilities. You will be responsible for ensuring the smooth day-to-day running of the service, including managing staff, supervising support plans, and maintaining relationships with service users and their families.
Key Responsibilities
- Supervise and support a team of support workers to deliver person-centred care and support to adults with learning disabilities
- Ensure compliance with relevant legislation, policies, and procedures
- Develop and implement effective support plans to meet the needs of service users
- Maintain accurate records and reports as required
- Collaborate with other teams to ensure seamless delivery of services
What We Offer
We offer a competitive salary of £28,371 per annum, 20 days annual leave (rising to 25 days after 5 years), flexible shift patterns, and a range of benefits, including a pension scheme and employee assistance program.
Requirements
- Degree in Social Care or related field
- Experience working with adults with learning disabilities
- QCF Level 3 in Health and Social Care
- Able to work shifts, including sleep-ins and waking nights
- Flexible and adaptable approach
