About the Role

We are seeking a highly skilled and dedicated Community Support Worker to join our team in Plymouth. As a CFO Evolution Specialist, you will play a vital role in supporting individuals in a community environment, helping them to overcome challenges and reintegrate into their communities.

Key Responsibilities

• Manage a caseload of participants, providing guidance and support to help them achieve their goals
• Collaborate with volunteer peer mentors and/or mentors to ensure effective case management and coordination of risk
• Contribute to a learning environment that enables colleagues to develop their skills and experience
• Ensure effective case management and coordination of risk, with a focus on safeguarding adults and children
• Meet individual performance targets and contribute to team performance targets

Requirements

• IAG Level 3 desirable
• Experience managing a caseload of participants desirable
• Excellent verbal, non-verbal, and written communication skills
• Ability to motivate and sustain engagement of individuals
• Knowledge of inputting and retrieving electronic data and maintaining computer-based information systems
