Parkinson's Specialist Practitioner

We are seeking a highly skilled and experienced Parkinson's Specialist Practitioner to join our team at Sirona care & health CIC. As a key member of our clinical leadership team, you will play a vital role in the development and delivery of our Community Parkinson's Practitioner Service.
Key Responsibilities- Provide high-quality specialist advice and support to individuals with Parkinson's disease, enabling them to manage their condition independently and improve their quality of life.
- Assess, plan, implement, and monitor care for people with Parkinson's, ensuring that care plans are person-centred and based on best practices and evidence.
- Develop and maintain strong relationships with patients, families, and healthcare professionals to provide specialist advice and support.
- Contribute to the education and development of other healthcare staff, ensuring that they have the necessary skills and knowledge to provide high-quality care.
- Participate in clinical governance and audit activities, including supporting clinical trials and ensuring the application of evidence-based practices.
- Demonstrable experience of working at Advanced Practice level within your clinical area.
- Experience of leading care planning and managing multi-disciplinary or integrated pathway teams.
- Ability to take overall responsibility and accountability for the delivery of high-quality care.
- Experience of medicines management within your own practice, either through an independent prescribing qualification or through the use of patient group directions or patient-specific directives.
