Reablement Officer Role Overview

We are seeking a skilled Reablement Officer to join our team at Service Care Solutions. As a Reablement Officer, you will play a vital role in supporting individuals to achieve maximum skills for independent living in the community.

Key Responsibilities

* Support the team leader in ensuring that people are enabled to achieve or regain maximum skills for independent living in the community
* Provide direct management support to the Support Workers
* Develop and implement goal plans and intervention plans that focus on outcomes
* Promote independence and empowerment to support people

Requirements

* Good understanding of current health and social care policies
* Ability to undertake specialist reablement assessments
* Ability to develop and implement goal plans and intervention plans that focus on outcomes
* Level 2 or 3 NVQ or other form of comparable training

Working Hours

* 35 hours a week
* 2 shifts: morning (7.30am - 3.30pm) and evening (1pm - 9pm)
* Weekend and bank holidays working

Rate

* £24 umbrella an hour (£19 PAYE and hour)
