Compassionate Clinical Practitioner for Patient Care

The NHS is seeking a compassionate and skilled Clinical Practitioner to provide high-quality patient care in a medical setting. As a key member of the healthcare team, you will be responsible for assessing, planning, developing, implementing, and evaluating programmes that promote health and well-being.
Key Responsibilities:
- Assess, plan, develop, implement, and evaluate individual treatment plans for patients with known long-term conditions.
- Proactively identify and manage treatment plans for patients at risk of developing long-term conditions.
- Provide sensitive communication styles to ensure patients are fully informed and consent to treatment.
- Communicate effectively with patients and carers, recognising the need for alternative methods of communication.
Requirements:
- Degree in Nursing or relevant field.
- Extensive experience in patient care and management.
- Strong clinical skills, including immunisations, venepuncture, and dressings.
- Ability to work effectively in a team environment.
Estimated Salary: £42,000 - £52,000 per annum.
