We are seeking an experienced Nurse Practitioner to join our team at HC-One Scotland, where you will play a vital role in delivering high-quality, person-centred care to individuals living with dementia. As a Specialist Dementia Care Community Nurse Practitioner, you will work closely with our multidisciplinary team to provide comprehensive care and support to our residents.
Key Responsibilities- Deliver person-centred care and therapeutic interventions to individuals living with dementia, focusing on their unique needs and preferences.
- Collaborate with our team of nursing, social care, psychology, and wellbeing practitioners to ensure seamless care delivery.
- Conduct holistic nursing assessments and develop individualised care plans to meet the complex needs of our residents.
- Provide education and training to colleagues on best practices in dementia care, promoting a culture of excellence and continuous improvement.
- Participate in quality improvement initiatives and contribute to the development of our care services.
- Post-registration clinical experience in dementia care, with a minimum of 2 years' experience in a similar role.
- Strong knowledge of dementia care policies and guidelines, including the National Dementia Strategy.
- Excellent communication and interpersonal skills, with the ability to work effectively with individuals from diverse backgrounds.
- Ability to work autonomously and as part of a team, with a strong focus on collaboration and partnership working.
- Commitment to ongoing professional development and continuous learning.
