About the Role

We are seeking an experienced and enthusiastic Care Manager to join our Adults Community Team. As a key member of our team, you will be responsible for leading a dedicated team of support workers in providing high-quality care to adults with learning disabilities.

The successful candidate will have a proven track record of leading and managing teams, with a strong understanding of care planning processes and a commitment to delivering person-centred care. You will be responsible for overseeing reporting, leading on community events, and working as part of the senior leadership team to maintain a high quality of care.

Key Responsibilities
  • Lead a team of support workers in providing high-quality care to adults with learning disabilities
  • Oversee reporting, including medication records, health and safety, and quality assurance
  • Lead on community events and activities
  • Work as part of the senior leadership team to maintain a high quality of care
Requirements
  • Qualified and experienced social care professional
  • Proven track record of leading and managing teams
  • Strong understanding of care planning processes
  • Commitment to delivering person-centred care
  • Ability to work as part of a team and build strong relationships with colleagues and service users
