Job Title: Youth Transition Coach

We are seeking a skilled and dedicated Youth Transition Coach to join our team at Youth Connect South West. As a key member of our team, you will play a vital role in supporting disadvantaged young people to achieve better post-16 outcomes.

About the Role:

The successful candidate will work closely with selected young people in Years 10 and 11, developing positive relationships and creating action plans to achieve the project's goals. You will also collaborate directly with schools, families, employers, and other partner agencies to ensure the effective delivery of sessions and promote a clear understanding of the project among all stakeholders.

Key Responsibilities:
  • Develop and implement tailored 1:1 and group sessions to support young people in achieving their goals
  • Collaborate with schools, families, and other partner agencies to ensure effective delivery of sessions
  • Work closely with young people to develop positive relationships and create action plans
  • Monitor and evaluate the effectiveness of sessions and make recommendations for improvement
Requirements:

To be successful in this role, you will need:

  • GCSE Maths and English Grade C/4 or above
  • Level 4 Information, Advice and Guidance qualification
  • OR Level 3 Youth Work qualification as recognised by the JNC Report for Youth and Community Workers
  • OR Another relevant Level 3 or above qualification i.e. Social Work, PGCE with relevant experience of direct work with young people from a diverse range of backgrounds and with a variety of needs
