Active Care Group is a leading provider of neuro and mental healthcare services in the UK. We are seeking a highly motivated and experienced Care Coordinator to join our team at Murdostoun Brain Injury Rehabilitation Centre in Wishaw.

The successful candidate will be responsible for planning, organising and implementing a social, recreational, and leisure activity programme in partnership with our existing Activities Co-ordinators. This will involve individual and group activities within the unit and the wider community.

We are looking for an individual who is able to work as part of a multi-disciplinary team and under their own initiative. The post holder will be responsible for:

  • Planning and delivering a range of activities to promote social interaction, independence and well-being
  • Working with residents to identify their interests and preferences and develop a tailored activity programme
  • Collaborating with other members of the multi-disciplinary team to ensure a cohesive and effective approach to care
  • Monitoring and evaluating the effectiveness of the activity programme and making recommendations for improvement
