Four Seasons Health Care is seeking a compassionate and skilled Care Home Assistant Practitioner to join our team. As a key member of our care team, you will play a vital role in delivering high-quality person-centered care to our residents.

About the Role

We are looking for an experienced and qualified Care Home Assistant Practitioner who can support our Registered Nurses in ensuring that all residents receive quality medical and personal care. Your responsibilities will include monitoring the wellbeing of each resident, identifying training needs of the team, and conducting staff supervisions and appraisals.

Your Responsibilities

  • Assist the Registered Nurse in ensuring that all residents receive quality medical and personal care.
  • Monitor the wellbeing of each resident and identify any changes which impact on care and care plans.
  • Support the Home Manager in implementing policies, procedures, and business objectives.
  • Conduct staff supervisions and appraisals, ensuring they meet external regulator guidelines.
  • Act as a focal point for contact with residents, their relatives, and visitors, as well as health and social care professionals.

What We Offer

In return for your dedication and hard work, we offer a competitive salary of £28,000-£32,000 per annum, depending on experience, plus benefits including pension scheme, life assurance, and childcare vouchers.
