Clinical Pharmacist Position

We are seeking a highly skilled Clinical Pharmacist to join our team at East:MK Primary Care Network. As a key member of our multidisciplinary team, you will play a vital role in ensuring safe, evidence-based, and cost-effective prescribing practices.
Main Responsibilities- Work collaboratively with the multidisciplinary team to provide patient-centered care.
- Conduct structured medication reviews to proactively manage patients with complex polypharmacy.
- Provide clinical medicines expertise to patients and practice teams.
- Improve prescribing practices across the PCN through clinical audits and quality improvement initiatives.
- Interface with community and hospital colleagues to resolve issues and enhance patient care.
East:MK Primary Care Network serves a population of approximately 52,000 patients across four practices in Milton Keynes. Our network is committed to delivering high-quality care, working collaboratively with NHS and community services to improve patient access and care.
Person Specification- Vocational 4-year master's degree in pharmacy.
- 1 year's pre-registration training.
- Registered with the General Pharmaceutical Council.
- Postgraduate diploma/MSc in clinical or community pharmacy, or equivalent.
- Non-medical prescriber registration.
- Evidence of continuing professional development.
- Covid vaccination training.
- Good critical appraisal skills.
- Experience of patient consultation.
- Experience of working with GPs.
- Good understanding of primary care structure and general practice.
- Good clinical knowledge.
