We are seeking a highly skilled and experienced Nurse Assessor to join our team as a CHC Nurse Assessor / Case Manager. This is a patient-facing role that requires a strong understanding of Continuing Healthcare (CHC) and the ability to deliver high-quality care to patients across all aspects of care.
Main Responsibilities- Timely review of all Continuing Healthcare/Funded Nursing Care patients, ensuring accurate assessment and support.
- Providing continuing support and advice to family and carers in complex situations, requiring excellent interpersonal and communication skills.
- Facilitating smooth patient flow across all aspects of the patient's journey, including liaison with primary care, secondary care, community venues, and specialist services.
- Supporting the publication of regular reports on progress within a given caseload of patients/sphere of service.
- Exercising professional judgement when communicating information on behalf of the CHC service and applying associated criteria.
- Delivering formal and non-formal education programs to Community and hospital staff, including nursing/residential homes, GPs, community nursing, and therapy staff, relating to the assessment and management of CHC patient groups in line with the latest research, evidence, and DOH/NICE guidance.
- Ensuring timely 'End of life' Care and reviews, following national guidance for clients Fast Tracked through the CHC system.
- Registered Nurse (Children, Mental Health, Learning Disability) qualification.
- Experience of working in a team and on own initiative, with a strong understanding of case management and transferrable skills.
- Excellent communication and interpersonal skills, with the ability to work with a wide range of health and social organisations.
- Previous experience in a similar role in the public sector.
