Role Overview
Airedale NHS Foundation Trust is seeking a highly skilled Clinical Outreach Specialist to join its Digital Care Hub team. This 5-month role will focus on supporting the delivery of digital services across Airedale and Craven, with a strong emphasis on outreach and collaboration with various teams within the Acute Trust and Community Services.

About the Job
The successful candidate will provide high-quality, evidence-based remote clinical assessment and establish plans of care for patients accessing services in the DCH. They will also support palliative/end-of-life patients and their families/carers who access the Goldline service via telephone. Additionally, they will participate in early intervention, ensure timely responses to remote monitoring alerts, and work collaboratively with acute, primary care, and community teams in managing sick patients.

Key Responsibilities
* Provide expert remote clinical assessment and develop personalized care plans for patients
* Support palliative/end-of-life patients and their families/carers
* Participate in early intervention and utilize clinical assessment skills
* Ensure timely responses to remote monitoring alerts
* Collaborate with multidisciplinary teams to manage patient care

Requirements
* RN/Project 2000 Adult Branch / BSc (or equivalent) AHP Qualification e.g. OT/PT/SALT
* Non-Medical Prescribing/Advanced Clinical Assessment (or willing to work towards)
* Current NMC/HCPC registration
* ENB 998/C & G 730/SLiP Educational qual.
* Over 2 years of Band 5 clinical practice

Salary and Benefits
