Care Manager Professional

We are seeking a highly skilled and dedicated Registered Care Manager to join our team at Invest Solutions Limited.
Key Responsibilities:- Complete social care assessments and undertake casework of residents in the community and other care settings.
- Commission care services according to assessment outcomes and eligibility in relation to the Care Act.
- Participate in the duty rota - this is the single point of contact for referrals for Adult Social Care.
- Undertake crisis intervention where enquiries indicate the need for immediate risk management.
- Carry out home visits and consult with service users, carers, and relevant agencies and professionals.
- Alert the Registered Care Manager to any safeguarding concerns.
- Ensure assessments and support plans are carried out in a person-centered and outcome-focused way.
- Advise on the use of personal budgets and direct payments.
- Assist customers to maximise their independence.
- Undertake any necessary adjustments or to cease care packages in accordance with both the organization's Eligibility Criteria and Operational Guidance and the needs of the service user.
Requirements:
- Strong understanding of the Care Act and its implications.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Strong problem-solving and analytical skills.
- Ability to maintain confidentiality and handle sensitive information.
