About the Role

We are seeking a highly skilled Digital Transformation Lead to join our team at Havering Liberty PCN. As a key member of our team, you will play a crucial role in driving transformation and improvement across the PCN, enhancing patient access and experience, as well as operational efficiency and staff job satisfaction.

Key Responsibilities
  • Lead transformation and improvement initiatives across the PCN, working closely with staff at practice and PCN level to understand demand, capacity, and activity.
  • Develop and implement quality improvement (QI) processes to support the implementation of change and assess the impact of agreed transformation.
  • Review and improve the PCN digital maturity to support population health management, improvements in care quality, and PCN operational efficiency.
  • Drive the planning and prioritization of changes that will improve delivery of care and patient and staff experience within the practices.
  • Build relationships across the PCN to develop consensus for improvements and ensure business and clinical staff are fully engaged throughout the process of selection, implementation, and deployment of change.
  • Provide hands-on capacity and support to practices in implementing and sustaining agreed changes.
  • Support staff to identify and participate in relevant training and/or professional development opportunities that will support achievement of agreed improvements.
  • Support the effective adoption of national and local initiatives, including integrated working at neighbourhood and system level to improve access to patient services.
  • Advise on innovative opportunities and support practices to maximize benefits from innovation, through the sharing of learning and best practice.
Requirements
  • Good understanding of the digital agenda within the NHS and social care/care sector.
  • Excellent working knowledge of Microsoft Office and the ability to navigate online tools and apps.
  • Experience in digital/IM&T project management or transformation experience in a health or social care environment.
  • Experience in project delivery through the entire project lifecycle from initiation to delivery.
  • Experience of promoting and expanding use of digital systems.
  • Experience of providing support to staff in development and use of digital or IT skills.
  • Experience of data collection and using tools to measure the impact of services.
