About the Role:

This is an exciting opportunity for a Healthcare Professional to work as a Community Health Care Assistant, providing personal care and support to individuals in their own homes. You will work as part of a close-knit team, supporting clients with daily living tasks, promoting independence, and maintaining dignity.

Key Responsibilities:

  • Providing personal care and support to clients in their own homes
  • Assisting with daily living tasks, such as bathing, dressing, and medication management
  • Supporting clients with mobility issues and helping them maintain their independence
  • Working collaboratively with other healthcare professionals to ensure comprehensive care

Requirements:

To be successful in this role, you will need:

  • A Level 3 Diploma in Health and Social Care or equivalent
  • Experience working in a healthcare setting, preferably in a community-based role
  • Excellent communication and interpersonal skills
  • A valid driving license and access to a vehicle (for mileage reimbursement)
