Job Overview

The NELFT NHS Foundation Trust is seeking a highly skilled and experienced Advanced Clinical Practitioner to join our Rapid Response Team in Waltham Forest. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care and making clinical decisions in a fast-paced and dynamic environment.

Main Duties

  • Work collaboratively with health and social care professionals to provide comprehensive patient care
  • Lead clinical decision making and undertake holistic assessments of patient needs
  • Triage referrals from other agencies and coordinate intensive nursing care
  • Develop and implement comprehensive care plans with patients and their families
  • Participate in quality improvement initiatives and contribute to service development

Requirements

  • Registered Nurse (Part 1 of NMC register) or Health & Care Professions Council registration
  • Non-Medical Prescriber qualification
  • Postgraduate level study or equivalent experience
  • Significant previous post-registration experience in relevant key areas at Band 6 or above
