Job Description

As a Clinical Health Coach, you will work closely with patients, physicians, and other healthcare professionals to develop and implement personalized care plans. Your primary responsibility will be to assess patient status, identify areas for improvement, and provide coaching and education to help patients manage their chronic conditions.

You will also collaborate with internal stakeholders to ensure seamless delivery of care, and participate in ongoing training and professional development to enhance your skills and knowledge.

Requirements and Qualifications

  • Current RN licensure and successful completion of disease management orientation program.
  • Minimum of 2 years full-time equivalent of direct clinical care to the consumer.
  • Basic typing/computer knowledge with minimum keyboarding speed of 35 WPM.
  • Previous disease management or case management experience preferred but not required.
