Key Responsibilities:

  • Provide personal assistance to the individual with daily tasks and activities\
  • Offer emotional support and companionship to alleviate feelings of loneliness and isolation\
  • Maintain the individual’s safety and well-being in the community by accompanying them on outings and errands\
  • Collaborate with healthcare professionals to ensure the individual receives high-quality care and support\
  • Communicate effectively with the individual, their family, and other stakeholders to ensure seamless care coordination\
  • Develop and maintain strong relationships with the individual and their family to ensure continuity of care\
  • Become familiar with the individual’s preferences, routines, and needs to provide tailored support\
  • Adapt to changing circumstances and priorities while maintaining a focus on the individual’s needs\
  • Maintain confidentiality and adhere to data protection policies at all times\
  • Contribute to a safe and supportive environment that promotes the individual’s dignity and autonomy\
  • Participate in training and development opportunities to enhance skills and knowledge\
  • Ensure compliance with relevant legislation and regulations\
  • Foster a culture of inclusivity and respect within the team and with the individual\
  • Continuously evaluate and improve the quality of care provided\
  • Stay up-to-date with industry developments and best practices\
  • Actively promote the values and principles of the organisation\
