About the Role

We are seeking a skilled and compassionate Reablement Officer to join our team at Walsall Council. As a Reablement Officer, you will work closely with individuals in their own homes to support and encourage them to develop or regain skills to live independent and fulfilling lives.

Key Responsibilities:

  • Work with individuals to identify and address their needs and goals
  • Develop and implement personalized support plans to promote independence and well-being
  • Collaborate with the therapy team to deliver bespoke support and contribute to ongoing reviews
  • Provide guidance and support to individuals to help them achieve their goals

Requirements:

  • Confident and effective communicator with excellent interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and analytical skills
  • Passion for working collaboratively with residents and colleagues to promote independence and well-being
