Job Title: Clinical Practitioner

We are seeking a motivated Clinical Practitioner to join our Urgent Community Response (UCR) and Frailty Virtual Ward (VW) team delivering care to patients closer to home and preventing avoidable hospital admission across North Hampshire.

Key Responsibilities:
  • Deliver specialist practice in UCR working closely alongside the Consultant Practitioner, Senior Clinical Practitioner, Advanced Clinical Practitioners, Trainee ACP's and other ECP's supporting the delivery of care in the home settings of our population.
  • Develop strong working relationships with the wider systems teams which may include community geriatrician, specialist doctor, GPs, and other MDT colleagues.
  • Provide a high standard of complex, enhanced care for patients, using enhanced levels of clinical judgement, skills and knowledge.
  • Consult with patients, their family, carers and the multi-professional team to undertake assessments of patient need and devise and evaluate complex care plans related to their field of expertise.
Requirements:
  • Appropriate professional qualification (nursing or allied health professional) that allows registration with the NMC or HCPC
  • Independent non-medical prescribing or pharmacology level 7 profession relevant qualification
  • Able to demonstrate a good command of the English Language both written and verbal
  • Computer Literate in both data entry and communicating via IT - emails and report
