Job Overview:
The successful candidate will have the opportunity to work in a fast-paced environment, collaborating with multidisciplinary teams to deliver person-centred care. You will be responsible for assessing patients' nutritional needs, developing and implementing care plans, and monitoring progress.
Your Key Responsibilities:
- Assess patients' nutritional status and develop personalized care plans.
- Work closely with healthcare teams to ensure seamless care delivery.
- Monitor patient progress and update care plans as required.
- Supervise and support dietetic assistants to ensure high-quality care.
