Job Summary

Key Responsibilities:
  • Provide personal care and support to patients, respecting their autonomy and dignity at all times.
  • Actively promote recovery-focussed care with patients, working collaboratively with other healthcare professionals to achieve set goals.
  • Lead patient-focus groups and activities, promoting social interaction and engagement.
  • Accurately assess and record observations on patients' mental and physical state, escalating any concerns as necessary.
  • Collaborate with multidisciplinary teams to ensure timely and accurate recording of patient information.
Requirements:

To be successful in this role, you will require:

  • GCSEs in Maths and English or equivalent qualifications.
  • A qualification at Level 2 or in Health and Social Care or another relevant subject.
  • Experience working in a healthcare setting, preferably in a similar role.
  • An understanding of confidentiality and equal opportunities principles.
  • The ability to work with vulnerable adults and demonstrate empathy and compassion.
