Job Title: Community Practitioner
We are seeking an experienced Community Practitioner to join our team at East Kent Hospitals University NHS Foundation Trust. As a Community Practitioner, you will be responsible for providing high-quality patient care in a community setting.
Key Responsibilities:
- To provide a safe and supportive environment for patient care.
- Pre-assessing nursing in clinic to ensure optimal patient outcomes.
- To work collaboratively with the multidisciplinary team to deliver effective patient care.
- To participate in the development and implementation of care plans that meet the individual needs of patients.
- Ability to coordinate surgical admissions lounge and areas of pre-assessment with ease.
- Risk management issues and postgraduate qualifications are highly desirable.

Requirements:
