About the Role

We are seeking a highly skilled and experienced Adult Social Care Specialist to join our Dedicated Mental Health Team. As a key member of our team, you will play a vital role in providing excellent Adult Social Care services to residents in Cheshire West and Chester.

Key Responsibilities
  • Undertake complex Mental Health Act assessments, implementing appropriate courses of action or treatment
  • Work closely with local people, families, and carers to provide support and guidance
  • Develop and implement effective support plans, ensuring the best possible outcomes for service users
  • Collaborate with other teams and services to ensure a seamless and integrated approach to care
  • Contribute to the development and implementation of policies and procedures, ensuring they are aligned with best practice and legislation
Requirements
  • Professional Social Work/OT/Mental Health Nurse/Psychologist qualification
  • AMHP qualification (postgraduate certificate on master's level) with current warrant to act as an AMHP
  • Experience of managing high-risk, complex cases and safeguarding Adults
  • Understanding of services essential to meeting the needs of people with Mental Health needs
  • Ability to communicate effectively with customers, carers, and families
  • Ability to cope positively with change and promote positive changes to make a difference
  • Awareness of the importance of multi-disciplinary working and an integrated approach
