About the Role:

Creative Support is seeking an experienced practitioner to provide care and support to individuals with learning disabilities in our supported living and outreach service in Blackpool.

Key Responsibilities:

  • Develop and implement person-centred care plans to promote independent living skills and community inclusion.
  • Identify and access activities that promote valued and meaningful lifestyles for service users.
  • Provide respectful personal care, emotional, and practical support to service users.
  • Collaborate with service users, families, and the multi-disciplinary team to maximise service user outcomes.
  • Implement Creative Support principles, policies, procedures, and guidelines to ensure the service meets CQC requirements.
  • Supervise and support staff to best meet service user needs and preferences.
  • Support service users with behaviour that challenges services and implement positive behaviour support.

Requirements:

  • Essential experience in a similar role.
  • Access to a car.
  • Ability to work flexibly, including evenings and weekends.
