Mental Health Practitioner

We are seeking a skilled and compassionate Community Mental Health Practitioner to join our team at Surrey and Borders Partnership NHS Foundation Trust. As a key member of our Community Mental Health Recovery Service, you will play a vital role in providing assessments, evidence-based treatment, and psychological interventions for adults with severe and enduring mental health issues.
Your primary focus will be on promoting recovery and independence, developing self-management strategies, and encouraging individuals to achieve their full potential in the community. You will work closely with our multidisciplinary team to assess and plan care, responding to changes in service users' health and well-being.
To be successful in this role, you will require a strong understanding of complex clinical decision-making, excellent communication skills, and the ability to work effectively with diverse stakeholders. You must also have a full and valid driving license and access to a vehicle for work purposes.
Main Responsibilities- Coordinate and monitor the delivery of person-centered healthcare to designated service users, utilizing complex clinical decision-making where necessary.
- Collaborate with the multidisciplinary team to assess and plan care, responding to changes in service users' health and well-being.
- Develop and maintain professional relationships with staff, other professionals, and agencies to enhance recovery and promote a culture of collaboration.
- Accurately record all contacts with service users in case notes, adhering to SABP record-keeping policies.
- Assess and plan care in accordance with evidence-based practice, policies, and procedures, while managing risk effectively.
- Understand the range of risks potentially faced by the service user group and utilize clinical supervision and appraisal to enhance performance and development.

Person Specification- Postgraduate diploma level or 5 years equivalent experience in mental health
- 2 years' experience post-qualification in a relevant role
- Awareness of personal and professional boundaries
- Clinical supervision training
- Experience working with people with dual diagnosis or service
- RMN/RN (Mental Health)
- Successful completion of Mentorship course
