We are seeking a skilled and compassionate Relief Team Leader to join our team at Praxis Care Group. As a Relief Team Leader, you will play a vital role in supporting the needs of individuals with mental ill health, autism, learning disabilities, and dementia.

About the Role

This is an exciting opportunity to be part of a new service and be an integral part of the staff team that will aim to provide the best quality of life for the person we are supporting. As a Relief Team Leader, you will have the opportunity to work with a dedicated staff team who support individuals to learn and develop their skills to aid in their independence.

Key Responsibilities
  • Support the needs of individuals with mental ill health, autism, learning disabilities, and dementia
  • Work collaboratively with the staff team to provide high-quality support services
  • Develop and implement care plans that meet the individual needs of our service users
  • Ensure that all staff members are equipped to carry out their role through comprehensive training
