About the Role

We are seeking a compassionate and dedicated Care Assistant to join our team at Connected Health. As a Care Assistant, you will play a vital role in providing high-quality care and support to individuals in their own homes.

Your Key Responsibilities:

  • Deliver personal care and daily living tasks to service users, in accordance with their individual needs and preferences.
  • Work collaboratively with service users, their families, and healthcare professionals to ensure seamless care and support.
  • Develop and maintain professional relationships with service users and colleagues from diverse backgrounds.
  • Communicate effectively with the service team, including regular updates on service user conditions and circumstances.
  • Participate in training, team meetings, and supervision sessions to enhance your skills and knowledge.

Essential Criteria:
