About the Role

We are seeking an experienced Community Healthcare Specialist to join our team at Buckinghamshire Healthcare NHS Trust. This is a fantastic opportunity to work in a dynamic and innovative environment, delivering expert interventions and rehabilitation to patients in their own homes.

The successful candidate will have a strong background in physiotherapy or occupational therapy, with experience of working in a community setting. They will be able to manage complex cases, develop and maintain partnership working, and motivate staff to deliver high-quality care.

As a key member of our Rapid Response & Intermediate Care (RRIC) team, you will be responsible for providing triage, comprehensive specialist assessment, review, and evaluation of patient needs. You will also plan programmes of care to promote health gain and maximise independence, actively case managing patients through their journeys.

We offer a competitive salary of £43,041 - £58,944 per annum, depending on experience, plus a range of benefits including flexible working opportunities, generous annual leave entitlement, and access to NHS discount schemes.
