Job Description
Social Care Connections is seeking a skilled Community Healthcare Practitioner to join our team and contribute to our mission of delivering high-quality care services to our clients.
This is a permanent position offering a salary range of $70,000 - $90,000 per annum, commensurate with experience.
Key Responsibilities
- Deliver high-quality care to clients in a community setting
- Develop and implement care plans that meet the needs of clients
- Collaborate with other departments to ensure seamless service delivery
- Monitor and evaluate the quality of care provided
Requirements
- Degree in Nursing or related field
- Minimum 2 years' experience in community healthcare settings
- Excellent communication and interpersonal skills
- Able to work in a fast-paced environment
