Job Overview
The Moray Council is seeking a highly skilled Community Care Specialist to join its Integrated Community Care Area Team. This role involves working with unpaid carers and their families to assess and meet their needs, ensuring they receive the necessary support to maintain their independence.
Salary and Benefits
As a Community Care Specialist, you will be rewarded with a competitive salary of £43,204.20 per annum, equivalent to £22.92 per hour, for a 36.25-hour workweek. Additionally, you will have access to various benefits and opportunities for professional growth within the Council.
Key Responsibilities
Our ideal candidate will possess experience in a Health/Social Care setting, with a demonstrable understanding of main issues facing Community Care. They will also hold relevant professional qualifications in Social Work, be SSSC Registered, and possess effective communication skills, both written and oral.
Some of the key responsibilities of this role include:
- Undertaking holistic assessments of unpaid carers' needs and commissioning packages of care;
- Liaising with other local authority services, partner agencies, providers, and the Carer Support Service to seek to meet needs in the most efficient and appropriate way;
- Providing direct service and support to carers and collaborating with colleagues within the Health and Social Care Teams;
- Maintaining case records, providing information for performance indicators, unfulfilled procedures, and statutory duties in line with national and local policy practices and procedures.
Requirements and Qualifications
To succeed in this role, you must be able to work effectively in partnership with others, possess strong interpersonal skills, and be able to manage change and prioritize tasks. You will also need to be competent in using IT equipment and software, with good keyboard skills, and have the ability to travel efficiently between various work locations within Moray.
Additionally, our ideal candidate will have:
- Relevant professional qualification in Social Work;
- SSSC Registration;
- Effective communication skills (written and oral);
- Ability to manage change;
- Effective record-keeping skills;
- Ability to work to deadlines and under pressure;
- Ability to manage own workload;
- Competent use of IT equipment, software, and good keyboard skills;
- Assessment and organisational skills;
- Finance skills;
- Ability to negotiate and liaise in relation to care management;
- Ability to work flexibly to meet the demands of the service;
- Ability to reconcile demands/wishes against needs and financial pressures;
- Ability to work in an open plan/shared office;
- Ability to work in different offices dependent on service requirements.
Estimated Salary Range
The estimated annual salary for this role is £43,204.20, based on a 36.25-hour workweek and an hourly rate of £22.92.
Working Environment
This role will involve working as part of a multi-professional team within the Moray Council's Integrated Community Care Area Team. As such, our ideal candidate will be comfortable working in a collaborative environment and be willing to adapt to changing circumstances.
