Job Overview

Buckinghamshire Healthcare NHS Trust is seeking a highly skilled and experienced Advanced Community Practitioner to join our team. As a key member of our Thame Adult Community Health Care Team, you will play a vital role in providing high-quality care to patients in the community.

Main Responsibilities
  • Conduct comprehensive assessments of patients' health needs and develop effective care plans to meet their complex requirements.
  • Proactively identify patients who are at high risk of unplanned hospital admission and work with multidisciplinary teams to intensively case manage their care.
  • Develop and implement systems and processes to support intensive case management within the team and with partners across the health system.
  • Work collaboratively with other healthcare professionals to provide seamless care to patients and their families.
  • Contribute to the development of policies and procedures to improve patient outcomes and experience.
Requirements
  • Registered Nurse with a degree in a relevant field and postgraduate clinical training in areas such as elderly care, long-term conditions, and palliative care.
  • Experience of working as an Advanced Clinical Practitioner or Case Manager in a community setting.
  • Strong leadership and management skills, with the ability to motivate and inspire teams to deliver high-quality care.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
  • Ability to work autonomously and make decisions in a fast-paced environment.
