**Job Overview:**

Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust is seeking a highly skilled Learning Disability Practitioner to join our Children and Young Peoples Services team.

The successful candidate will have experience in working with children and young people with learning disabilities and complex needs, as well as excellent risk assessment and management skills.

Main Responsibilities:
  • Assess and manage casework of young people and their families with substantial and complex needs.
  • Develop individualised Care Plans with regular reviews to monitor progress and adapt as necessary.
  • Coordinate Treatment Planning & Interventions, Reviews, and Discharge Planning.

The estimated salary for this position is around £45,000 - £65,000 per annum, depending on experience.
