Clinical Care Practitioner

Athena Care Homes is seeking an experienced Clinical Care Practitioner to join our team. As a key member of our care staff, you will play a vital role in delivering high-quality care and support to our residents.
As a Clinical Care Practitioner, your primary responsibility will be to work closely with our care team to assess, plan, implement, and evaluate individualized care plans. This will involve providing expert advice and guidance to our care staff to ensure that our residents receive the best possible care.
The estimated salary for this position is £28,000 - £38,000 per annum, depending on experience. If you are passionate about delivering outstanding care and support, we encourage you to apply.
Job Requirements:
- Provide expert advice and guidance to our care staff to ensure that our residents receive the best possible care.
- Work closely with our care team to assess, plan, implement, and evaluate individualized care plans.
- Develop and maintain effective relationships with our residents, their families, and other healthcare professionals.
- Monitor and report any changes in a resident's condition to the relevant authorities.
- Ensure that our residents' care plans are regularly reviewed and updated to reflect their changing needs.
- Contribute to the development and implementation of new policies and procedures to improve the quality of care and support we provide.
- To actively engage with residents in conversation and meaningful occupation related to their lifestyle choices at a level and pace that values the individual and respects their dignity and communication differences.
- To respond in a timely way to residents who are distressed (calling out, calling for help, knocking or making noises that suggest the need for support) or seek assistance if you feel unable to respond appropriately.
Health and Safety:
- Attend and complete all training and qualifications (statutory, mandatory and developmental).
- Ensure Senior Carers or RNs are informed of any incidents in the home.
- Ensure storage of medications are in line with company policies and procedures
- Ensure risk assessments are completed within the home and safe working practices are being followed.
- Ensure equipment and environment is safe within the home
- You will be required for this role to potentially lift heavy objects and operate lifting equipment (with training).
