Job Overview

We are seeking an enthusiastic and dedicated Clinical Lead to join our multidisciplinary team at Swansea Bay University Health Board. The successful candidate will be responsible for supporting the Team Manager in the day-to-day clinical management of a designated older peoples' Secondary Care Community Mental Health Assessment and Treatment Team.

Main Duties of the Job

The post holder will be responsible for care-coordinating a caseload, ensuring the philosophy of person-centred care is shared by the team and the patient/carer is central to the care planning process. They will also support junior staff within their roles and aspirations, and lead a team in a supportive manner.

Key Responsibilities

  • Assess, plan and implement care for older people with functional mental health illnesses and complex health issues.
  • Provide clinical supervision to staff and students.
  • Work closely with carers, families and partner agencies to facilitate care planning and discharge planning.

Requirements

  • Excellent understanding of assessment and management of risk.
  • Effective communication and interpersonal skills.
  • Ability to positively engage in problem-solving and overcoming challenges to create personal and team resilience.
