AYG Care is partnering with a reputable domiciliary care company in North London to recruit a newly qualified health and social care professional to work as a Care Coordinator. This is an excellent opportunity for someone fresh out of their health and social care studies to grow and develop within a supportive and dynamic environment.
Key Responsibilities- Assist in the day-to-day management of client care services.
- Ensure that all care provided is tailored to the individual needs of clients.
- Build and maintain strong relationships with clients and their families.
- Support the management team in handling complaints and implementing solutions.
- Collaborate with CQC inspectors during inspections to maintain high standards.
- Keep all client documentation, including care plans and risk assessments, up to date.
- Coordinate carer placements and manage contracts.
- Conduct regular spot checks and ensure the care planning system is current.
- Handle accident and incident reports appropriately.
- Maintain regular communication with both carers and clients to ensure the delivery of quality care.
- Newly qualified in health and social care, with eligibility to work in the UK.
- Confident driver with access to company pool cars for client visits.
- Strong communication and organisational skills.
- Ability to manage emails and maintain an efficient filing system.
- Willingness to liaise with professional bodies and inspectors as needed.
