Key Responsibilities
  • Carry out holistic reviews of patients with asthma and COPD, identifying areas for improvement and developing targeted care plans.
  • Initiate investigations for patients on respiratory pathways, ensuring timely and effective treatment.
  • Implement and evaluate individual care plans, ensuring patients have a clear understanding of their condition and how to manage exacerbations.
  • Identify and manage patients at risk of developing respiratory disease, providing proactive support and guidance.
  • Recommend and prescribe inhaled therapies in line with local and national guidance, aiming for good symptom control.
  • Carry out spirometry and interpret results, providing accurate and contemporaneous records of patient consultations.
  • Produce accurate records of patient consultations, consistent with legislation, policies, and procedures.
  • Advise and support patients on vaccinations, including influenza and pneumonia vaccines.
  • Carry out home visits for patients who are unable to attend the surgery, providing personalized care and support.
  • Offer expert advice on the management of patients' long-term conditions, ensuring optimal health outcomes.
