The East London NHS Foundation Trust is seeking a highly skilled and experienced Clinical Expert Coordinator to join their team. As a key member of the multidisciplinary team, you will play a crucial role in ensuring the smooth discharge of patients from hospitals.

Main Responsibilities

  • ">
  • Undertake 'in reach' triage assessments for patients on acute wards to ensure rapid discharge.">
  • Coordinate and chair IDH Clinical Discussion meetings with system partners and discharge hubs.">
  • Liaise with Discharge Coordinator Teams and other system partners to identify and facilitate patient discharge.">
  • Maintain relationships with Hospital Social Work Teams and other system partners to ensure patient needs are met.">
  • Participate in shift work, covering various times, including weekends and Bank Holidays.">

Requirements

  • Experience in screening and triaging referrals and discharge planning.">
  • Leadership qualities and ability to manage a team.">

Salary and Benefits

We offer a competitive salary of £56,300 per annum, plus benefits, including a pension scheme and annual leave entitlement.
