Teesside Hospice Care Foundation is seeking a dedicated and compassionate Palliative Care Support Worker to join our team in the In-Patient Unit.

About the Role:

The primary focus of this role will be to work within the multidisciplinary team, assisting in providing palliative nursing care for patients under the direction of a registered nurse. As a Palliative Care Support Worker, you will play a vital role in enhancing the quality of life for our patients and their families.

Key Responsibilities:

  • Assist in providing high-quality patient care, adhering to hospice standards and protocols.
  • Work collaboratively with the multidisciplinary team to develop and implement individualized care plans.
  • Support patients and their families in coping with end-of-life care, offering emotional and practical assistance.
  • Maintain accurate records and reports, ensuring timely communication with the healthcare team.

Requirements:

  • A minimum of 6 months' experience in a healthcare setting, preferably in palliative care.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and staff.
  • Able to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently.
  • Compassionate and empathetic towards patients and their families, with a strong commitment to delivering exceptional care.
