About the Role

We are seeking a highly skilled Care Coordinator to join our team in South East England. As a Care Coordinator, you will work closely with adults with care and support needs, their families, and carers to promote independence, wellbeing, and informed choices.

Key Responsibilities
  • Deliver person-centred care and support tailored to individual goals and preferences.
  • Take a holistic approach, supporting families and carers to enhance the lives of those in their care.
  • Build strong community links and partnerships to provide effective support networks.
  • Conduct comprehensive assessments, create care plans, and review them regularly to meet personal outcomes.
  • Manage crisis situations and lead safeguarding enquiries.
  • Maintain high standards through personal reflection, supervision, and training.
  • Work flexibly across locations and teams, ensuring an effective, resilient service.
Requirements
  • Degree or Diploma in a relevant field (e.g., Social Work, Nursing, or Occupational Therapy).
  • Up-to-date registration with your professional body.
  • Competence at the new practitioner level of the Social Care Capabilities Framework.
Estimated Salary Range

£35,000 - £45,000 per annum.
