Job Opportunity: Care and Support Coordinator

We are seeking a highly motivated and experienced Care and Support Coordinator to join our team, working on a part-time basis to support adults with complex needs in the community and in their own homes.

About the Role:

This is an excellent opportunity to work with a dynamic and supportive team, making a positive difference in the lives of individuals with complex learning needs. As a Care and Support Coordinator, you will be responsible for coordinating support plans, ensuring high standards of care, and promoting independence.

Key Responsibilities:

  • Coordinate support plans with service users, families, and multidisciplinary teams
  • Ensure high standards of care and promote independence
  • Support with daily living activities, such as meal preparation and household management
  • Promote participation in community activities and maintain high standards of nutrition, health, safety, cleanliness, and personal hygiene
  • Work in a way that treats everyone equally and fairly, whilst promoting service users' rights and dignity

Requirements:

  • Satisfactory clearance of an Enhanced Disclosure & Barring Service (DBS) check
  • Proof of right to work in the UK
  • Ability to demonstrate awareness/understanding of equal opportunities and other people's behavior, physical, social, and welfare needs
  • Good report writing skills
  • Demonstrate professionalism and ability to travel effectively to different locations

Estimated Salary: £26,000 - £32,000 per annum, depending on experience
